Monitoring system used by Horizon Home Health.

Horizon Home Health, located at 250 North Fairgrounds Road, offers in-home nursing care, therapy services and aide care. Their staff is available 24 hours a day to care for families.
Horizon Home Health accepts medicare, medicaid, most third-party insurances and private pay.
As part of their care, Horizon Home Health offers something no other agency in the area can, daily home monitoring of their patients with the HomMed monitoring system.
A day for their home health aides is also varied. Bobbi Jo Dimick begins her day early. She may assist one client with personal care, then continue on to do a homemaking visit, or provide respite care for someone who needs to get out for awhile, and leaves their family member in Bobbi’s capable care. “It took me two years to decide to become a CNA. I am so glad that I finally did. I like all of the people that I take care of. I like going home at night, knowing that I did something good for someone during the day, something that helped,” says Bobbi.
Horizon Home Health has brought new technology to Carbon and Emery Counties. It is here to serve the community with their exclusive HomMed monitoring system which allows them to provide continuous medical supervision while people remain in the comfort of their home.
This is achieved by collecting vital signs at scheduled report times through a series of short easy steps via the HomMed home monitoring system. It then automatically transmits your information to the central station where your home care nurse can assess your vital signs. The home unit is a vital sign monitoring and communication device that measures weight, blood pressure, heart rate, temperature and oxygen saturation. Additional adaptors can be added to take blood sugars or other specific measurements recommended by your physician. Regular monitoring allows clinicians and your physicians to assess your condition and react quickly to any concerns.
When used as part of an overall health plan the HomMed system can reduce emergency room visits, hospital admissions and improve your quality of life. Patients agree that the HomMed monitor is easy to use and not time consuming. Each day, a gentle, friendly voice guides them through the simple, three-minute procedure to collect all of their vital signs and asks customized subjective questions to which they answer yes of no. These questions are to assist further evaluation of their conditions and quality of life measurements.
